Our integrated care model

Social support and behavioral health, working hand in hand.

Social support

Resources, case management and practical help

Behavioral health

Connections to qualified mental health providers

Coordinated support for whole-person well-being

Care works better when it connects the dots.

Housing instability, financial hardship, language barriers, unemployment, family difficulties and immigration-related stress can affect emotional well-being. SCIRC helps address practical barriers and connect people with appropriate behavioral health care.

A clear distinction

SCIRC provides social services, resource navigation and care coordination. Clinical behavioral health services, such as therapy and psychiatric care, are provided by appropriately licensed professionals.

With an individual’s consent, SCIRC can coordinate with healthcare providers to support continuity of care. No one should have to navigate complex systems alone.
